SR 5th ed. Validation "Domain must be specified"

Ok my first forum post on here .
One of my players wanted to add a third Eye to his cyber skull.
we figured out how to add the additional amount to the skull and bought an extra eye.
But now we have a red validation that says "cyber skull (obvious) – additional eye mount : domain must be specified" if I turned the eye mount off of being get a validation error " you need an additional eye mount for each additional cyber"
is anyone else having errors like this am I just missing something or is this a bug any help would be appreciated
and yes I searched the forum first and couldn't find anything
I had looked but could have missed it thank you

If you're purchasing gear that needs to be customized and you're in advancement mode, be very careful to purchase the gear using the Customize button not the Add button, and be careful to fill out everything before clicking OK.

In this case, you should describe where the extra eye is mounted.

Thank you
I did it 3 times and finally figured out if you have to write something in both blanks "name"and "description"
so we named it "back of head" and description was "rear head mounted "

That's gotten me a few times never really understood why the required domain can't be edited after creation and you have to make an entirely new item to add it.
