Taking Fatigued as an example, this text cpuld be appended to the pop-up:
'For each hour of rest ypu can attempt a Con test (DC 10) to recover. Recovery is automatic afer 10 hours of sleep. The Leadership feat can also be used to remove ths condtion.'

I just looked for some of these, and they weren't all in the conditions section like I expected. I don't know the rulebook well, so it would take a long time for me to go through and find them all; could you compile a list of the additions that you want? If you can get that together, I can add them really quickly.
