Making a new Form of the Dragon Adjustment

Our campaign has created a 9th level Form of the Dragon IV spell.

I made a copy of the Shadow Chemosh 3rd degree spell adjustment but am trying to figure out how to add the appropriate dragons to it's list.
 
So far I've been able to make a copy which has the text, but I need to be able to add the relevant dragon forms and change the modifiers and attack forms that would be granted.
