How to activate a form for the Untamed Druid?

Unfortunately, the search didn't give me any results for this, maybe because it is so obvious and I am just blind :)
How can I select and activate the animal form I want to use during play?
And if that's for any reason not directly possible, what is the most practical way to have the stats available I use in the new form?
