Er, the header has only appeared at the top of each page since AT LEAST
V1.4 was released about a year ago. It may have been longer ago. There were
lots of people who complained that rosters were much longer than necessary
due to the repetition. That's why the headers were removed and support for
option descriptions as footnotes was added.
