I am unsure how to add a Spell as an Action. Any help would be greatly appreciated. Please see the image for the details of the spell.

Like all actions within HL 5e you will need to make a racial/class special as a action and add in the info which you want from the file. There is a option to tag it as an action. The user UI will then show the thing as a ranged spell attack action. If you can't change the race/class through some means like the archetype to add this action then you will probably need to make an adjustment to add it to the creature. If a custom creature then just bootscrap the thing to it.

You can even do this with Natural Attacks on a monster. Just add the wProperty.Spell tag. If it's a ranged attack, it will automatically say "Ranged Spell Attack". Check here for more info on Natural Attacks.
