Mythic Archmage ability: Transformative Familiar

How do I select what item/weapon my familiar changes into, and how do I add them as my weapon to my character sheet?
 
Pretty sure you just add a copy of the weapon to your character sheet and use it when the ability is activated (drop it to the ground so it doesn't affect your weight)
