The journal seems to be adding a random block no information entry that isn't on Hero Lab but is on the print preview.

Is this a bug, something I'm doing wrong, or ???

The second entry is just an empty entry that can't be found in Hero Lab itself, but only in the Printed Version...
 
It seems to be a phantom Prestige Award entry. I have gotten rid of it by making the character a Pathfinder Society character, adding a Prestige Award point, then deleting the entry.
 
Worked for me... Did you actually add prestige to the entry and then delete it, or just add and delete? I did the first and it worked for all three of the pc files that I have for my players.
