Form of the Dragon

I added an adjustment for Form of the Dragon and when asked to choose a race or new form it says nothing to choose from and none of the stats that should change are changed. So it doesn't add the +4 size bonus to Strength, a +2 size bonus to Constitution, a +4 natural armor bonus, fly 60 feet (poor), darkvision 60 feet, a breath weapon, and resistance to one element. also don't gain one bite (1d8), two claws (1d6), and two wing attacks (1d4) attacks to the character. I just got my Green Dragon form armor but ran up against a dragon and tried to shift and got none of the changes in the Armory or in-play areas.

I checked all levels of Form of the Dragon and none of them added anything.

I did check and I own the companion # 11 which has the Dragon Slayer's handbook which should manage all of these and make the needed changes

I just tested this and it works fine for me. I then turned OFF my 'Bestiary' sources and I get the same message as you did - "nothing to choose from".
It appears that you require at least Bestiary 1 to be able to activate the spell adjustment correctly.
If you believe that having the Dragon's Slayers handbook should allow this to work, then report this as a BUG.

Thanks Dami, I didn't even think to check the Bestiary sources, that fixed the problem. I was trying to figure it out in the middle of the fight and keep track of what others were doing as well. I'm all set now.
