In no particular order.

1 - A simpler way of handling natural spellcasting, so that added levels of sorceror or whatever do not have their progressions thrown off.
2 - The ability to modify Animal Companions/Familiars/Special Mounts, for example the Natural Bond feat increasing the bonuses applied to a druid's companion, or a prestige class that stacks for companions.
3 - The ability to create custom wands or potions in a similar fashion to what is done for scrolls. I know this is being worked on for Pathfinder at least.
 
4 - A unified mechanism for Smites and other commonly used special abilities (I am thinking specifically of the many different Improved Grab specials there are). This would allow easy checking for pre-requisites, such as with the Earth's Embrace feat (which is currently having to check for each of the 14 improved grab abilities).

5 - Javelins and other thrown weapons to be sorted out. Personal Pet Peeve that Javelin is being treated as a melee weapon by default. I've been asking for this one for years it feels like!
