Scripted Live Name different levels

I am trying to create a script that adds a live name to a class ability at different levels.

I have tried looking at the UC monk's FOB which kinda does what I want, but I don't understand the scripting well enough to write it at multiple different levels. I could be / probably am overlooking something obvious...

Specifics: The class ability grants a multiple effects at different levels and want to have those effects detailed using the live name script.

Without specifics about what you want to include there's too many ways to go about this to just list them all.

I did list specifics at the end of my post, however, I will elaborate a bit.

Specifics: I have a class ability that grants 3 different abilities at different levels.

In the class ability tab on the character sheet, I want it to look like this:
2 class ability (extra movement)
3 class ability (extra attack)
6 class ability (opponent rerolls attacks)
11 class ability (extra attack)
