Yes, it is for pathfinder Curse of the Crimson Throne - and believe it or not, I'm still playing this module and using the item.
It should have 3 checkboxes; darkvision 60ft, or perception, or see invisibility.
I tried to do it myself, but alas I could not get it to do more than 2 checkboxes. Any help is appreciated.
