Regarding chapter 6, how does one configure those options?

When they are available (they are under review by the Lone Wolf powers that be), they can be selected individually on the Configure Your Hero panel.

The chapter on "fixing" mythic, was a big draw to the Mythic Hero's Handbook, for our group.
