Creating a new Disease

I am trying to create a new disease for my Wrath of the Righteous game. I am using the unchained system. I have created the disease in hero lab and have entered all the required text. When I add the disease, it will show all the text and cycle through the levels of the disease as expected.

The disease progresses down the physical and mental tracks simultaneously, similar to Mummy Rot, but it does not skip the Latent/Carrier stage.

How do I get hero lab to apply all the conditions? I think I am missing the eval scripts, but I have no idea how to write a script to do this. I tried watching a youtube tutorial, but I have no experience in coding. The guys kept saying it was easy, but I do not think so.

Can someone help me with the eval script or tell me what else I am supposed to do to get this to work?
