Enhancing Weapons/Armor

Greetings!

I want to equip myself with a simple set of Chain Mail +3, and a Great Sword +2.

How do I do that? It seems like it should be a simple thing, but I can't find out where/how to do it. Happy to follow instructions if they're listed somewhere, but in my preliminary searches I couldn't find the answer.

On both the armor and weapon tables, when adding a new magic item, the very first item in the list of things you can purchase is a customizable item.
